Schorr Vision, PLLC ("Schorr Vision," "we," "us") operates schorrvision.com. This policy explains what information we collect when you visit this website, how we use it, who we share it with, and the choices you have.
This policy covers the website only. Information you share with us as a patient, including anything you tell us during a consultation, exam or procedure, is protected health information governed by our Notice of Privacy Practices and by HIPAA. If anything in this policy conflicts with the Notice of Privacy Practices as it applies to patient information, the Notice controls.
1. Information you give us
When you fill out our contact form, we collect your name, email address, phone number and the procedure you are interested in. When a referring doctor uses our referral form, we collect the information entered on it. When you request an appointment through our scheduling tool, Klara, you provide the information Klara asks for to book and confirm the visit. When you call or text us, we keep the phone number you reach us from and any voicemail you leave.
You are not required to give us any of this information. Without it, we cannot respond to your request or schedule a consultation.
2. Information collected automatically
Like most websites, we collect certain information automatically when you visit, using cookies, pixels and similar tools. This includes your IP address, approximate location, device and browser type, the pages you view, how long you stay, the site that referred you, and whether you clicked to call us or to book an appointment.
The tools that collect this information are Google Analytics 4 and Google Tag Manager, which help us understand how visitors use the site; Google Ads, which helps us measure whether our advertising brings people to the site; and the Meta Pixel, which helps us measure and improve our advertising on Facebook and Instagram.
These tools are operated by Google LLC and Meta Platforms, Inc. under their own privacy policies. We have configured them to measure website activity, not to identify individual patients.
3. How we use information
We use the information we collect to respond to your questions and schedule consultations; send you the communications you asked for; operate, secure and improve the website; understand which pages and advertisements are useful; and comply with legal obligations.
We do not sell your personal information, and we do not share it with third parties for their own marketing.
4. Who we share information with
We share information with companies that provide services to us, including website hosting, appointment scheduling, phone and messaging, and email. These providers may use your information only to perform services for us. Where a provider handles patient information, we have a business associate agreement with them as HIPAA requires.
We share the automatically collected information described in Section 2 with Google and Meta so their tools can function.
We may also disclose information when the law requires it, to protect our rights or the safety of others, or in connection with a sale or reorganization of the practice.
5. Health information and this website
We treat the procedure interest you select on our contact form as sensitive. We use it only to prepare for your consultation and to route your request to the right person. We do not send it to Google, Meta or any advertising platform.
Please do not send detailed medical information through the contact form. Use the secure messaging in Klara or call us at 702.702.2020.
6. Text messages and calls
By giving us your phone number, you agree that we may call or text you about your request and your appointments. Message frequency varies. Message and data rates may apply. Reply STOP to any text to opt out, or HELP for help. Your consent to receive texts is not a condition of receiving care.
