If you’ve been researching vision correction, you’ve almost certainly come across LASIK. You might have heard of SMILE. But EVO ICL? Probably not, and that’s exactly why I want to tell you about it.
EVO ICL is one of the most exciting procedures in refractive surgery today. It delivers outstanding visual outcomes, works for patients who can’t get laser procedures, and has a patient satisfaction rate of 99.4% in FDA clinical trials. Yet most people don’t even know it exists until a surgeon brings it up.
Let me change that.
What Is EVO ICL?
EVO ICL stands for Implantable Collamer Lens. It’s a small, flexible lens, about the size of a soft contact lens, that’s permanently implanted inside your eye to correct your vision. Unlike LASIK or SMILE, EVO ICL doesn’t reshape or remove any corneal tissue. Your corneas stay completely intact.
Here’s how it works: Through a tiny incision that’s so small it seals on its own, I carefully position the lens between your iris (the colored part of your eye) and your natural crystalline lens. The implanted lens then does the focusing work, correcting for nearsightedness and astigmatism with exceptional precision.
Your natural lens stays right where it is. The procedure is additive, we’re enhancing your eye’s optical system rather than permanently altering its structure.
What Makes EVO ICL Different from Other Vision Correction Procedures
There are a few things about EVO ICL that I think are worth highlighting, because they set it apart in meaningful ways.
It doesn’t touch your cornea
LASIK corrects vision by removing corneal tissue. That’s precisely why corneal thickness is such a strict requirement. EVO ICL bypasses this entirely, we’re not touching the cornea at all, which opens up the procedure to a much wider range of patients. If you’ve been told your corneas are too thin for LASIK, EVO ICL may be exactly what you’ve been looking for.
It’s excellent for higher prescriptions
Laser vision correction has limits. Beyond certain prescription levels, the amount of tissue removal required starts to push against safety margins. EVO ICL can correct a significantly wider range of nearsightedness, making it the preferred option for patients with moderate to severe myopia.
It’s gentler on dry eye patients
LASIK works by creating a corneal flap, which disrupts corneal nerves and can reduce tear production, sometimes significantly. For patients who already have dry eye or are prone to it, this is a real concern. EVO ICL doesn’t disrupt corneal nerves, making it a much friendlier option for those with tear film concerns.
It’s reversible
This is one of the more remarkable aspects of EVO ICL that patients consistently appreciate: if your prescription changes significantly later in life, the lens can be removed or exchanged. LASIK permanently reshapes your cornea, that change is not reversible. EVO ICL offers a level of flexibility that laser procedures can’t match.
Visual quality is exceptional
I want to be careful not to make it sound like LASIK produces poor results, it doesn’t. But many EVO ICL patients describe a crispness and clarity of vision that surpasses what they experienced with glasses or contacts. Night vision outcomes are particularly notable. The EVO ICL also includes built-in UV protection.
What consistently surprises my ICL patients isn’t the distance vision, they expected that. It’s the little things they didn’t know they were missing. The texture of leaves on a tree across the street. Reading highway signs at night before their passenger can. Colors that feel more saturated. High prescription patients have often spent decades looking through thick lenses that shrink and distort the world, and the ICL removes all of that at once. More than one patient has told me it’s like going from standard definition to 4K. I perform LASIK with outstanding results every week, but there’s a particular kind of delight I see in my ICL patients that is genuinely its own thing.
What to Expect: The EVO ICL Procedure
The EVO ICL procedure is performed in our in-office surgical suite. You’ll receive numbing eye drops, no general anesthesia needed, and mild sedation to keep you comfortable. The procedure itself typically takes about 15 to 20 minutes per eye.
Most patients notice improved vision within hours. By the next day, the majority are seeing clearly enough to resume most normal activities. Full healing takes a few weeks, during which you’ll use prescription eye drops and attend follow-up appointments to ensure everything is healing as expected.
Recovery is smooth for most patients. Clinical data shows that at six months post-op, 98.5% of EVO ICL patients achieved the same or better vision than they had with their best glasses correction. More than half achieved better vision than glasses ever gave them.
Is EVO ICL Right for You?
EVO ICL candidacy requires a specific anterior chamber depth (the space between your cornea and natural lens) as well as appropriate overall eye health. We assess this thoroughly at your consultation using detailed imaging and measurements.
The procedure is FDA-approved and has been performed worldwide since the 1990s. The EVO version, which includes a central port that eliminates the need for a pre-operative laser procedure that older ICL models required, was approved in the United States in 2022.
